Forma Supply Co is currently a men’s apparel subscription service that provides high-quality and affordable monthly supplies of Forma Supply Co brand apparel.
DO I CHOOSE MY MONTHLY SELECTIONS?
Forma Supply Co’s focus is to provide you the newest styles of apparel while also getting you outside your comfort zone every month with apparel that fits your style. No more wasting time on escalators or in checkout lines; All you’ve got to do is open the package that comes to your door.
IS THE CLOTHING THAT I RECEIVE REALLY WORTH DOUBLE IN VALUE?
Forma Supply Co has been in the garment game for over a decade before its name came along. If you compare our quality and pricing with other similar brands in retail, we are on par with leading brands in the marketplace. If you go to a contemporary fashion retailer in a mall or online, you will expect to pay anywhere from $30-40 for single t-shirts. Our monthly packages are in line with those price points, however, we deliver double the value by giving our members a minimum of 50% off these retail prices.
HOW DO THE SHIRTS FIT?
Our shirts are slightly fitted but we’re not “the Man-zier.” We wanted to curate the right fit that keeps you comfortable and looking fresh.

AM I COMMITED?
As a Forma Supply Co member, you can pause or cancel your account at any time after your first month. We ask our members to contact us via live chat or update/cancel within their account before the 10th of the month if they would like to pause or cancel.
HOW AM I GETTING CHARGED?
At the moment Forma Supply Co accepts Visa, MasterCard, Discover and American Express. We do not accept cash, personal checks, money orders, or PayPal.
The first month’s payment is charged right after you press the “Submit Order” on the Secure Checkout page. For every month after that, Forma Supply Co members are charged on the 15th.
CALIFORNIANS: WHY AM I BEING CHARGED MORE?
Due to government changes in online sales policy, we are now forced to charge sales tax on all orders shipping to the state of California.
CAN I PAUSE A MONTH?
You can use your Skip Next Renewal feature within your account to pause your subscription at any time for up to 1 month. Your next month will go back to automatic renewal. For example, if you would like to pause the service for February, please update your account before the 31st of January to pause for that month. Your subscription will automatically renew for February’s billing and package.
You can find the Skip Next Renewal feature after login in here under “Update Your Information” by clicking “Edit.” On the next page, you’ll find the “Skip Next Renewal” button on the top. After confirming, you’ll see a date populated with the next charge date.
If you have any issues, just email email@example.com or chat us between the 16th & last day of the month, before 5 PM PST, and we will skip that requested month.
HOW DO I CANCEL MY SUBSCRIPTION?
If you wish to cancel your Forma Supply Co subscription, please use one of the below methods.
– Log into your account and message a live chat representative from your member dashboard from 9AM – 5PM PST Monday through Friday
– Cancel your account by logging into your profile and clicking “Edit” under “Update Your Information” then click “Cancel Subscription.”
Please Note: In order to opt out of the month’s package, cancellations must be done before the last day of the month. We look forward to hearing from you!
WHERE IS MY PACKAGE?
Typically, once your payment is made your package is picked within the week. We allow that time for our inventory professionals to allocate and confirm your order. Once the package is ready to go, we let the friendly USPS associate handle the rest.
Shipping takes anywhere from 7-10 business days from the 1st of each month depending on your geographic location.
Forma Supply Co is currently available in the U.S. Membership in other countries is in the works.

IF I WANT TO CHANGE MY STYLE PROFILE, CAN I DO SO?
We know style can change over time and people always want to diversify their looks and improve their wardrobe. All members have to do is log back into their profile and make adjustments in their profile. Or email our customer service team at firstname.lastname@example.org and they can definitely help you out.
RETURNS, REFUNDS AND EXCHANGES
HOW DO I EXCHANGE AN ITEM?
We do not accept returns. However, we do accept exchanges for size related issues. If something is too small or too big, please fill out and submit the Contact Form or chat us within your account, and we will get you something that fits correctly. All items must be received in unworn condition, with no stains, tears, or visible signs of use or they will not be accepted.
Items must be returned within 15 days of receipt. Your new package will be shipped to you once your old package is received and processed at our warehouse.

CAN I GET A REFUND ON MY SUPPLY?
If you have yet to receive your first supply of Forma, you may request a refund. The deadline to request a refund is the last day of the month in which you first purchased. All pre purchased supplies are non refundable once the first supply is received.
